To register with the surgery please call in at reception to collect a New Patient Registration Pack. This will then need to be completed and returned to the practice with 2 forms of ID, one Photo and one for a proof of address. You must reside within the Practice Area to register with the practice. All new patients will be offered an initial appointment for a health check with our Health Care Assistant in order for us to complete the registration process.
Named Accountable GP
Under the terms of the latest GP contract, every patient must be allocated a “named accountable GP.” This is an administrative requirement to ensure each patient has a responsible GP should they need them, but it does not restrict you from seeing any doctor of your choice within the Practice. Your named GP will not always be available, so if your needs are urgent, you may be seen by another doctor. Please note there is no need to telephone the Practice solely for this information, as our reception staff can confirm who your named GP is and, where possible, accommodate requests to change your allocated GP.
